RISE (REHOUSING IN SUPPORTIVE ENVIRONMENTS) DV SUPPORTIVE COMMUNITY CASE MANAGER- CARING FOR THE HUNGRY AND HOMELESS OF PEEKSKILL (CHHOP)
General Description: Our focus at CHHOP (Caring for the Hungry and Homeless of Peekskill) is housing and food equity. We operate a shelter for unhoused adults of all genders, including veterans, a collaborative housing and services support program for domestic violence survivors and their families, a permanent supportive hosing program for chronically homeless individuals living with a disability, a large “farm to pantry” food pantry, and a rental arrears and communities services support program for low-wage workers and immigrants. The RISE case manager is responsible for assisting domestic violence survivors and their families achieve and maintain safety and self-sufficiency
RISE Program: The Rehousing in Supportive Environments Program (RISE) is a collaborative program providing case management, housing, domestic violence, supportive services, and employment services to homeless domestic violence survivors and their families. This collaborative model was intentionally designed to provide “wrap-around” services in one program.
Principal Activities:
- Provide trauma informed case management services
- Conduct client intakes and assessments
- Develop measurable service plans
- Assist clients with overcoming barriers to sustaining permanent housing
- Support clients while keeping them focused
- Conduct required home/community visits
- Review budgeting practices with clients as an ongoing practice
- Develop person centered discharge plans
- Develop engaging methods of service delivery using a trauma informed approach
- Build strong community relationships
- Identify resources and community linkages to support client needs
- Remain current on domestic violence best practices and resources
- Assist with coordinating monthly case conferencing meetings and work to ensure the coordination of services.
- Interface and collaborate with Westchester County Department of Social Services and other service providers
- Other duties as required.
Qualifications:
- Bachelor’s Degree or equivalent professional experience in the human services or related field.
- Minimum of two (2) years working with survivors of domestic violence and/or clients experiencing trauma
- A record of successfully facilitating client change and goal achievement
- Ability to build supportive and respectful working relationships with staff, clients and community members
- Valid New York Driver’s License, car insurance, and personal vehicle
Skills and Abilities:
- Good oral and written skills
- Excellent communication skills
- Good time management skills, able to handle multiple tasks and assignments at the same time
- Self-starter, flexible and easily adaptable
- Ability to work well as a team member and independently
- Attentive to detail
- Ability to flex schedule
- Proficient in Internet research, E-mail, databases, and Microsoft programs

Salary Range: $59,000 , medical and other benefits
