Position Overview:
Caring Hands Palliative and Hospice Care Inc is looking for exceptional_ MSW, LCSW, and LMSW_ for per diem positions throughout Maricopa County. The Medical Social Worker plays an integral role in coordinating patient care, actively engaging in interdisciplinary assessments and crafting comprehensive care plans. Collaborating closely with physicians and the interdisciplinary team, the Social Worker ensures the effective implementation of hospice patients' care plans. Hospice Experience is highly desirable.
Key Responsibilities:
The Social Worker performs various tasks to support patients and families/caregivers in hospice care:
- Conducts psychosocial assessments and communicates findings to the interdisciplinary team, adapting plans if a safe residence isn't available.
- Conducts social evaluations, provides counseling for stress and coping difficulties, and plans interventions accordingly.
- Maintains clinical records for all referred patients.
- Offers information and referral services for practical and environmental needs.
- Provides information to patients, families/caregivers, and community agencies.
- Acts as a liaison between patients, families/caregivers, and community agencies.
- Collaborates with organization personnel to support patient care.
- Establishes and maintains contracts with public and private agencies for patient resources.
- Contributes to individualized care plans and attends interdisciplinary meetings.
- Assists the medical team in understanding social and emotional factors related to health and end-of-life issues.
- Actively participates in quality assessment and performance improvement activities.
- Performs other duties delegated by the Clinical Supervisor/Nursing Supervisor.
Qualifications:
- Completion of a bachelor's program in social work accredited by the Council on Social Work Education, meeting personnel qualifications outlined in §418.114(b)(3). MSW/LCSW preferred.
- Minimum of one year's experience in healthcare, with a preference for hospice experience, and a strong understanding of hospice philosophy and end-of-life issues. NEW GRADS WELCOMED TO APPLY.
- Preference for previous experience in hospice care.
- Demonstrated proficiency in verbal and written communication, as well as organizational skills.
- Possession and maintenance of current CPR Certification.
- Must hold a valid driver's license, have access to an insured automobile meeting state/organization requirement, and is in good working condition.
