What You'll Do:
- Conduct thorough patient assessments and develop individualized care plans.

- Submit concise and accurate home visit reports.
What You Bring:
- A valid LPN license in the state of Georgia.
- A minimum of one year of experience in home health care (preferred).
- Exceptional communication, organizational, and problem-solving skills.
- A compassionate and patient-centered approach.
- The ability to travel within a 50-mile radius.
- A valid driver's license and reliable transportation.
