Description

Reports to: Vice President, Research and Population Health with a dotted line to the
Chief of Behavioral Health.

Supervises: Transitions of Care Project Coordinator and Community Health Workers/Peer Recovery Specialists

Salary/Status: Exempt
Hours: 40 Hours/Week

Note: This position is grant-funded.

Minimum Requirements:
  • Bachelor’s or Master’s degree in Public Health, Public Policy, Public Administration, Health
Administration, Epidemiology, Social Sciences, Behavioral Sciences, Biological Sciences,
Management, or related field preferred.
  • Fluency in English; exceptional oral and written communication and professional
correspondence skills.
  • Experience or prior involvement in the development, improvement, or evaluation of health
programs/services in community settings.
  • Manage project scope effectively to ensure appropriate process are in place.
  • Strong analytical and reasoning skills.
  • Strong professional communication and correspondence skills.
  • Utilize project management tools and principles to define and manage project scope, document
and monitor timelines and deliverables.
  • Complete proficiency in Microsoft Office (Word, Excel, and PowerPoint) and Google Workspace
(Email, Calendar, Drive, Docs, Sheets, and Slides) applications, Zoom, Asana (to support
departmental leaders with project milestone tracking), Box, ShareFile.
  • Ability to demonstrate cultural humility with staff, teams, patients, stakeholders, and partners
representing diverse cultures, ethnicities, sexual orientations, gender identities, socioeconomic
statuses, educational backgrounds, worldviews, literacy levels, and languages.
  • Ability to work at a fast pace, manage numerous complex projects and teams, prioritize and
accommodate last-minute requests effectively, and tolerate pressure with relative equanimity.

Principle Responsibilities and Duties:
  • Coordinate a variety of evolving grants and initiatives focused on substance use recovery and
treatment, behavioral health/OBAT program expansion as well as transitions in care for justice-
involved populations.
  • Grants administration responsibilities will include the development and monitoring of workplans for
multiple grants, budget/fiscal management, data collection and reporting, execution of grant
objectives and deliverables, and funder communications/relations.
Exercise a high level of organization and attention to detail to ensure timely tracking, completion,
and submission of all grant deliverables.
  • Maintain positive relationships and responsiveness to grant project officers and technical
assistance providers, and ensure successful completion of all required meetings and
communications.
  • Collaborate with medical providers, and other clinical teams in the agency on the implementation
and evaluation of clinical and outreach programs focused on behavioral health and office-based
addiction treatment (OBAT) programs focused on providing recovery services to individuals with
substance use needs.
  • Manage partnerships and contracts with recovery/BH service consultants.
  • Work closely with the Quality Improvement (QI) leadership to lead QI efforts for grant-required
and agency-identified priority clinical areas, including quality measures pertaining to behavioral
health and substance use recovery efforts.
  • In collaboration with the Research and Population Health (RPH) and BH leadership, coordinate
and support grant submissions.
  • Attend and actively participate in grant-required meetings and training sessions.

For the Transitions in Care for Justice-Involved Populations program:o Participate in regular coordination meetings with the Hamden County Sheriff's Department staff to manage upcoming releases and care planning of justice-involved Individuals with SUD needs

Under the leadership of the Chief Behavioral Health Officer and the Chief Medical Officer, oversee expedited medical intake processes and ensure smooth implementation of care procedures.
o Provide strategic guidance and mentorship to Community Health Workers (CHWs) and Recovery Coaches.

Coordinate logistics for various programmatic collaborations (e.g., completion of Business
Associates Agreements, Data Use Agreements).
  • Report to and collaborate actively with departmental leadership; provide thorough communications and updates regarding programmatic successes and barriers; identify (and work collaboratively to address) clinical and operational inefficiencies and solutions.
  • Collaborate with data analysts to gather, analyze, and verify needed data for grant reporting, ensuring timely and accurate submission of all required metrics and performance outcomes.
  • Develop educational and outreach curricula and collaborate with support staff and other department teams to pilot and promote recovery support and wellness services in the agency.
  • Organize community outreach to promote the utilization of services relating to BH and OBAT.
  • Collaborate with cross-departmental teams (Interpreting/Translation Services, Marketing Department) on the development and dissemination of program-specific promotional and educational materials and campaigns.
Working Conditions:
  • The position requires prolonged periods of sitting at a desk and typing on a computer; the ability to stand
for prolonged periods of time; ability to perform moderate activities such as climbing stairs,
bending, stooping, reaching, and lifting up to 20 pounds at times; and potential exposure to
adverse outdoor environmental conditions.
