Description
Reports to: Vice President, Research and Population Health with a dotted line to the
Chief of Behavioral Health.
Supervises: Transitions of Care Project Coordinator and Community Health Workers/Peer Recovery Specialists
Salary/Status: Exempt
Hours: 40 Hours/Week
Note: This position is grant-funded.
- Bachelor’s or Master’s degree in Public Health, Public Policy, Public Administration, Health
Management, or related field preferred.

Principle Responsibilities and Duties:
- Coordinate a variety of evolving grants and initiatives focused on substance use recovery and
involved populations.

- Work closely with the Quality Improvement (QI) leadership to lead QI efforts for grant-required
health and substance use recovery efforts.
- In collaboration with the Research and Population Health (RPH) and BH leadership, coordinate

For the Transitions in Care for Justice-Involved Populations program:o Participate in regular coordination meetings with the Hamden County Sheriff's Department staff to manage upcoming releases and care planning of justice-involved Individuals with SUD needs
Under the leadership of the Chief Behavioral Health Officer and the Chief Medical Officer, oversee expedited medical intake processes and ensure smooth implementation of care procedures.
o Provide strategic guidance and mentorship to Community Health Workers (CHWs) and Recovery Coaches.

- Report to and collaborate actively with departmental leadership; provide thorough communications and updates regarding programmatic successes and barriers; identify (and work collaboratively to address) clinical and operational inefficiencies and solutions.
- Collaborate with data analysts to gather, analyze, and verify needed data for grant reporting, ensuring timely and accurate submission of all required metrics and performance outcomes.
- Develop educational and outreach curricula and collaborate with support staff and other department teams to pilot and promote recovery support and wellness services in the agency.
- Organize community outreach to promote the utilization of services relating to BH and OBAT.
- Collaborate with cross-departmental teams (Interpreting/Translation Services, Marketing Department) on the development and dissemination of program-specific promotional and educational materials and campaigns.
- The position requires prolonged periods of sitting at a desk and typing on a computer; the ability to stand
bending, stooping, reaching, and lifting up to 20 pounds at times; and potential exposure to
adverse outdoor environmental conditions.
