Key Responsibilities:
- Client Support: Act as the primary contact for clients and their families, providing guidance and support throughout their care journey.
- Communication: Maintain regular communication with clients to assess satisfaction and address any concerns or issues promptly.
- Care Coordination: Collaborate with care staff to ensure the implementation of individualized care plans and adjust services as needed.
- Problem-Solving: Identify and resolve client concerns, advocating for their needs with the care team and management.
- Education: Provide clients and families with information about available services, policies, and procedures.
- Documentation: Maintain accurate records of client interactions and service changes in the agency's database.
- Feedback Collection: Gather client feedback and contribute to quality improvement initiatives within the agency.
- Community Outreach: Participate in community events to promote agency services and build relationships with potential clients.
- Education: High School Diploma
- Experience: Prior experience in client relations, customer service, or healthcare is a plus.
