Caring Places Management is actively recruiting for a Resident Care Coordinator to join our team at our Assisted Living & Memory Care community, Awbrey Place, located in Bend, OR. The Resident Care Coordinator will report directly to the Administrator and fills a crucial role within the community by ensuring safe, high quality care for each resident.

Job Duties:
- Providing support to the RN in all duties of medical oversight as assigned within regulation
- Processing physician orders for second signature or pharmacy attention daily
- Tracking medication orders to ensure all prescriptions are filled in a timely manner
- Overseeing medication returns
- Assisting the RN with the completion and blocking of dummy sheets for MAR in a timely manner
- Preparing for RN signature and sending quarterly reports to physicians
- Communicating with physicians as directed by RN
- Processing relevant resident information into File Maker on a daily or as needed basis. Keep all resident File Maker records up to date.
- Make all transportation arrangements for resident doctor appointments daily and ensure that physician papers are prepared for appointments, given to residents, and collected from residents upon return.
- Archiving resident charts, medication orders, and physician faxes quarterly, observing all DHS archive guidelines
- Participating in the Resident Service/Care Planning process as directed by the Administrator and the RN
- Conducting monthly calibrations of glucometers
- Taking and recording daily vitals as directed by physicians
- Maintaining order in, and an accurate inventory of, all medical equipment/supplies for the facility
- Accompanying residents to physician appointments as necessary
- Providing support and training to care staff as directed by the Administrator and RN
- Scheduling all shifts, keeping the schedule posted a minimum of two weeks in advance
- Other activities as assigned
RCC will enhance the care and life quality for residents by:
- Having one-on-one visits with residents who have been ill, are reclusive, or who have shown a need for enhanced contact
- Assisting new residents with orientation to the facility and the services available
- Creating a calm and positive atmosphere using good communication skills, simple terms, and a reassuring tone of voice
- Communicating with family members and visitors as directed by the Administrator in a caring manner that protects the privacy of each resident
- Ensuring the safety and well-being of each resident by intervention and/or redirection when appropriate
- Providing personal care with ADLs as appropriate while striving to promote self-care, independence and dignity, and personal privacy
- Other activities as assigned

Requirements:Requirements:
- Prior experience as Lead Medication Aide preferred
- First Aid/ CPR Certified
- Must be able to respect the privacy and confidentiality of residents and have a clear understanding of residents’ rights
- Must be adept at all skills necessary to provide ADL in service to care staff, including: the ability to lift, push or pull up to 50 pounds, and the ability to bend, twist, stoop, kneel and reach
- Must have outstanding written and oral skills
- Must have the ability to provide leadership/instruction in all resident care functions
- Must possess a working, current knowledge of State ALF/RCF OARs
