Responsibilities
- Assess patient status through interview, observation, and physical assessment of actual problems and outcomes.
- Develop an individualized plan of care using a collaborative approach with the patient/family and healthcare team.
- Use appropriate interventions identified in the plan of care to achieve expected patient outcomes.
You will also coordinate the interdisciplinary care of patients through collaboration with team members and utilize additional resources when necessary.
