Position Summary :
The RN Intake Coordinator interprets and applies established admission criteria for Carle Home Health Services product lines to prospective patient referrals. Ensures the timely and efficient transmission of referrals to appropriate operational area and ensures behaviors that promote customer satisfaction. Hours may vary depending upon census and program need.
Qualifications :
EDUCATIONAL REQUIREMENTS College Diploma Nursing CERTIFICATION & LICENSURE REQUIREMENTS Registered Nurse (RN) Illinois and Basic Life Support (BLS). Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients. EXPERIENCE REQUIREMENTS One (1) year Healthcare / Medical ADDITIONAL REQUIREMENTS
SKILLS AND KNOWLEDGE Knowledge of home health, hospice, home infusion, case management, Medicare, Medicaid, federal and state accreditation standards, and experience in dealing with third party payers. Strong computer skills and Medical terminology.
