Carleton-Willard Village is a retirement community located in Bedford, MA. Services include supportive care, skilled nursing, rehabilitation, and memory care all in a village-type setting. Our 72 acre sanctuary of pristine woods, meadows, walking paths and gardens is just north of Boston.
Job Purpose:
Evaluates and assesses all assigned residents. Develops, implements and modifies treatment plans in accordance with physician orders and resident needs. Provides program planning, related and/or required documentation within the scope of Physical Therapy. Serves as an educator to residents, families, students, staff and self.
Essential Duties and Responsibilities:
- Possesses the assessment skills to obtain necessary data for effective treatment planning in accordance with standards of practice. Initiates referrals when appropriate that reflect effective use of standardized tests and measures.
- Possesses the knowledge and skill to identify an individual’s need for therapy intervention through the screening process.
- Provides Physical Therapy services to residents and appropriately modifies treatment plans and goals in a timely manner with respect to changes in a resident’s response to treatment or functional/medical status.
- Possesses the ability to assess and reassess resident’s functional mobility and readiness for discharge.
- Provides clinical information to members of the care team in a timely manner concerning resident‘s functional needs and response to therapy intervention.
- Prepares resident for discharge including home/community/facility and family needs, which includes recommendations for ongoing care and equipment needs.
- Completes clinical, administrative and interdepartmental documentation in a clear, concise timely and effective manner and in compliance with state, federal and intermediary guidelines.
- Implements and conducts treatment as outlined in a resident’s treatment plan. Coordinates the plan with resident, family and other team members.
- Recommends/provides or fabricates assistive/adaptive devices and trains residents, families and appropriate staff in use.
- Communicates effectively with all members of the interdisciplinary team in coordinating effective and efficient resident care.
- Directs and supervises Assistants and supportive personnel in the implementation of the treatment plan to maximize resident outcomes.
- Attends in-services and facility meetings as required.
- Responsible for professional development through self-study, continuing education and professional development obligations
- Documents and accurately reflects results of resident assessment, follow-up and termination of services.
- Adheres to department procedures regarding documentation of Physical Therapy services.
- Completes timely and accurate documentation of direct time, payroll, efficiency and clinical reporting in accordance with Carleton Willard Village standards.
- Adheres to policies and directives as developed for Carleton Willard Village; integrates company values and principles into daily performance.

Qualifications:
- Baccalaureate Degree from an accredited Physical Therapy program.
- Current licensure in the state as a Physical Therapist.
- Minimum of 3 years of experience in skilled nursing preferred.
- Reasoning ability.
- Strong communications skills.
Supervision of Others:
- Monitors treatment and documentation of Assistants.
- Monitors treatment and documentation of ancillary staff (restorative aides).
- Countersigns notes when appropriate.
