About the Role:
As a Care Coordinator, your responsibilities will include:
- Managing, developing, and reviewing the overall quality of care services provided by The Carlyle House.
- Conducting daily rounds with all nurse aids and initiating correction action as necessary.
- DIRECTING THE CARE NEEDS OF RESIDENTS ACCORDING TO THEIR CARE PLANS AND COMMUNICATING TO THE NURSING DEPARTMENT ANY OBSERVATION OF A RESIDENT'S MEDICAL CHANGE OF CONDITION.
- CONDUCTING MONTHLY (AND AS NEEDED) STAFF MEETINGS WITH ALL SHIFTS FOR PURPOSES OF PLANNING, COORDINATING, AND IMPLEMENTING DIRECT CARE SERVICE POLICIES AND PROCEDURES.
- COMMUNICATING WITH NURSE AIDS PROVIDING CLEAR, CONCISE INFORMATION.
