Job Summary
The Health and Wellness Coordinator is responsible for the management, development, and performance of Care Advocates personnel. This includes policy oversight, organizational planning, and direction for the delivery of care, as well as the development and monitoring of standards of care practices.
Key Responsibilities
- Manages, develops, and reviews the overall quality of care services provided by The Carlyle House.
- Conducts daily rounds with all nurse aids and initiates correction action as necessary.
- Directs the care needs of residents according to their care plans and communicates to the Nursing Department any observations of a resident's medical change of condition.
- Conducts monthly (and as needed) staff meetings with all shifts for purposes of planning, coordinating, and implementing direct care service policies and procedures.
- Communicates with nurse aids providing clear, concise information.
- Works closely with the Health Care Director to review all reports of accidents/incidents involving nurse aids and residents for completeness, accuracy of content, and appropriate signatures and interventions.
- Assists with any investigations involving allegations of abuse, neglect, misappropriation of resident property, and takes appropriate disciplinary action as necessary.
- Maintains up-to-date position control and master schedules.
- Completes performance appraisals prior to due date.
- Responsible to see that all Care Advocate appraisals are completed and sent to the Health Care Director and Executive Director by due date.
- Updates Horizon Payroll to reflect any employee change of status and monitors actual hours worked for each nurse aid.
- Performs HR function for all new hires, including ensuring physicals, drug screens, TB tests, training, and background checks are completed correctly and timely according to Ohio Department of Health.
- Supervises Care Advocate staff during normal shifts and meetings.
- Conducts daily rounds to ensure ADLs of residents are met.
- Communicates any change of condition or care needs of a resident.
- Conducts monthly Care Advocate meeting to discuss updates, ADLs, and memory care training.
- Ensures staff is aware of all state and federal regulations; including OSHA.
- Responsible for Corrective Action and appropriate follow-up.
- Develops Care Advocate schedules and ensures all shifts are covered to properly care for the residents.
- Responsible to work the floor if the shift cannot be covered.
- Updates and reviews all nurse aid forms to stay in compliance with rules and regulations.
- Conducts weekly review of care notes.
- Ensures care plans are up-to-date with correct information and followed accordingly with the Health Care Director.
- Trains and develops Care Advocates on the usage of Point of Care in Eldermark.
- Trains and develops nurse aids on Renaissance Neighborhood technologies.
- Develops and conducts training on the Renaissance Neighborhood Media Network and Snoozelen Therapy Room.
- Ensures Renaissance Media Network and Snoozelen Therapy Room is being utilized and tracked for effectiveness.
- Develops and implements aroma therapy and sensory motor stimulation skills in the Renaissance Neighborhood.
- Works with the Resident Events Director to develop, oversee, and personally conduct memory care programming for the Renaissance Neighborhood.
