Carlyle Senior Care of Florence is currently seeking an Admissions Coordinator to join our team! We are looking for a motivated and caring individual who shares in our desire to provide the best long term care possible. Admissions Coordinator Responsibilities Include:
- Maintain communication with hospital discharge personnel, administrators, other nursing homes, physicians, clinics, rehabilitation centers, and home health agencies. This may include visiting residents in the hospital or at home prior to transfer to the facility.
- Collect and respond to all admission inquiries and convert them into a new admission if possible. This may include gathering sufficient preadmission data to determine resident level of care and the ability of the facility to properly care for the resident.
- Provide facility tours to interested parties. This may include assisting families and residents in understanding the policies and procedures of the facility and charges made for services rendered.
- Coordinate all resident admissions by obtaining proper admissions paperwork. This includes completing the paperwork associated with the admission, including any legal documents such as power of attorney or advance directives.
- Work well with medical, nursing and administrative staff to ensure appropriate patient placement. This includes notifying appropriate departments when a resident is admitted and/or discharged.
- Maintain high occupancy rates with a proper mixture of Medicare, Medicaid, private insurance and private pay. This includes confirming that all insurance benefits meet the appropriate standard for admission.
- Coordinate with patient and family the adjustment to the facility on the day of admission.
- Maintain census trend reports and other data used to monitor the effectiveness of census activities.
