What are the responsibilities and job description for the MDS Nurse position at Carolina Care Health & Rehabilitation?
Carolina Care Health and Rehabilitation is currently hiring a dedicated and experienced full-time RN/LPN MDS Coordinator. This role is essential in managing the RAI process and ensuring that all Minimum Data Set (MDS), Care Area Assessments (CAAs), and Interdisciplinary Care Plans are completed accurately and on time. The MDS department works in close collaboration with our Regional and Corporate MDS support teams. Please note—this position does not involve on-call responsibilities or medication pass duties.
Key Responsibilities:
• Lead and oversee the Resident Assessment Instrument (RAI) process, ensuring compliance with all applicable guidelines, physician orders, company protocols, and regulatory standards.
• Develop comprehensive care plans with both short-term and long-term goals tailored to each resident’s needs.
• Coordinate and conduct meetings with interdisciplinary care teams, family members, and other stakeholders.
• Ensure all assessments are completed and submitted promptly.
• Supervise the entry and transmission of MDS data and confirm receipt of validation from state systems.
• Plan and lead care plan meetings and monthly Quality Measures (QM) sessions.
Qualifications:
• Active, unrestricted RN or LPN license in the state of employment.
• Previous experience with MDS processes, including submission and clinical reimbursement practices. Preferred
• Excellent communication, documentation, and critical thinking skills.
• Ability to prioritize tasks effectively and manage time well.
