Carolina Kidney Partners (CKP) is an organization designed to facilitate Value-Based-Care models for – and in alliance with – physicians’ groups. CKP is working with the Carolina kidney alliance (CKA) to manage and improve the health of patients with chronic kidney disease in South Carolina.
CKP offers a statewide one-stop solution for all patients with kidney disease, including chronic kidney disease, end stage renal disease and transplant services. CKP is focused on personal interactions with patients to improve health outcomes. Said another way- people engaged in the community, not apps or call centers, best provide care to patients with chronic kidney disease. The underlying premise is that only a clinically - driven system of care can improve the quality of health care in this cohort of patients.

Role Description
This position is a key team member supporting the Care Coordination efforts of our nephrology healthcare providers. The successful applicant will join our care coordination team as a full participant in preventive and proactive health care. Direct patient contact in the nephrology clinic providing care coordination for chronic disease kidney patients. Specific responsibilities include deploying the tools developed by CKP and CMS to capture key patient metrics of health care outcomes in patients. Along with capturing key metrics, this role engages directly with patients and assists them with the barriers they may have in adhering to their plan of care. This position works closely with a CKP RN counterpart as well as practice employees to ensure patients receive the right care at the right time, and in a proactive manner.
We believe value-based-care is not only the future payment model of patient-centered medicine, but the way to enable physician groups to deliver the most effective care possible to improve patient outcomes. Care coordination at the grass-roots level is key to this effort – and the role of our Care Coordination Analytics Specialist is a key to care coordination.
Responsibilities:
Deploy surveys & tracks patients attributed to one of our VBC programs
Understand & facilitate the data needs of our Care Coordinators and providers
Support RN Care Coordinators including:
Patient education on treatment modalities (kidney transplant, dialysis, supportive care)
Transplant education /appointment scheduling/follow-up care
Assistance patients with barriers to care and patient advocacy
Telephonic wellness check-in on patients
Consultation with the care team and other providers on patient needs
Work collaboratively with nephrologists and clinic team
Provide exceptional communication skills and customer service
Knowledgeable in data entry and data analytics
Excel competency is critical for data management
Provide care coordination in an office setting requiring in-person visits with patients in clinic
Coordinate with surgeons on referrals for dialysis access placement
Manage referrals being sent for kidney transplant evaluation
Excellent written skills for electronic health documentation
Other duties as assigned

Education and Work Experience Requirements: bachelor's degree required and prefer experience in health care setting
