Job Title: Home Care Nurse Coordinator
Reports to: Director of Home Care
Classification: Exempt
Summary/ Objective:
The Nurse Coordinator is responsible for the supervision of Home Care Caregivers in day to day operations and for participating in the orientation, initial education and initial assignments of newly hired Caregivers. The Nurse Coordinator provides the initial assessment of a Resident for Home Care, provides follow-up assessments as required by regulation, and follow up as needed to ensure Resident and Caregiver safety and effectiveness. The Nurse Coordinator communicates on a routine basis with other service providers at Carolina Meadows to ensure positive outcomes for Residents receiving Home Care.
Essential Functions:
1. Completes initial assessment of residents desiring Home Care services.2. Re-evaluates Home Care resident needs at least quarterly to assess changes in condition and to make necessary revisions to the Plan of Care initially established.3. Includes the resident, family and interdisciplinary team in the care planning process. Counsels and educates residents and their care team as needed.4. Supports the Agency Director with on-call duties as assigned.5. Documents all clinical information in the Electronic Medical Record.6. Supports the Home Care Quality Assurance/Performance Improvement Program7. Participates in on site urgent and non-urgent nursing care.8. For the purpose of inclement weather, is considered a member of the essential personnel team.9. Participates in the orientation process for new Caregivers.10. Participates in the ongoing education of all Caregivers.11. Complete Caregiver skills review and yearly Performance Review.12. Assures Caregivers recertification is complete and timely.13. Assists in scheduling process.

Supervisory Responsibility:
Responsible for supervising Caregiving staff in day-to-day operations, conducting periodic reviews and annual evaluations. Interviews and selects new staff. Make mandatory supervisory visits with the residents to ensure high quality care and accurate delivery of the plan of care and conduct additional visits as needed. Reports concerns to Agency Director. Obtains resident satisfaction information.
Required Education and Experience:
1. Graduate of an accredited school of nursing. 2. Two (2) to four (4) yrs. of long term care with geriatric medical service and transdisciplinary team experience.3. Two (2) years of home care or home health preferred.4. Current licensure as a licensed practical nurse or registered nurse in North Carolina.5. Current CPR certification and valid driver's license.
Preferred Education and Experience:
1. Experience in direct supervision of other staff including C.N.A. staff.2. Experience caring for those with dementia or cognitive impairment.3. Experience in home care or home health.4. Experience in managing staffing schedules.5. Experience in skilled nursing care.6. Experience in responding to medical emergencies.
