Position Overview
We are seeking a full-time Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) Team. This vital role involves delivering community-based clinical services to individuals with serious mental health challenges. The ACT Nurse will monitor medical and clinical aspects of services delivered, provide direct care, and work collaboratively with the multidisciplinary ACT team to ensure comprehensive client care.

The ACT Nurse will monitor medical and clinical aspects of services delivered, provide direct care, and work collaboratively with the multidisciplinary ACT team to ensure comprehensive client care.
Key Responsibilities
- Clinical Oversight:
- Monitor and oversee all medical and clinical aspects of client services.
- Provide medication monitoring and documentation.
- Deliver psychopharmacologic and medical services within the scope of practice.
- Service Coordination:
- Collaborate with the ACT team to monitor each individual’s clinical status and response to treatment.
- Consult with primary healthcare providers and other medical professionals on client care.
- Coordinate with state and community inpatient facilities for comprehensive care.
- Client Support:
- Conduct crisis assessments, symptom management, and intervention planning.
- Assist clients with activities of daily living, social and interpersonal skills, and leisure-time activities.
- Provide education, support, and consultation to clients’ families and natural supports.
- Documentation and Planning:
- Develop and assist with transition plans when clients move in and out of services.
- Complete required documentation, including treatment and goal plans, service notes, and activity records.
- Team Participation:
- Rotate crisis phone/on-call duties with other team members.
- Identify evidence-based or emerging best practice therapies for implementation by team staff.
- Licensure: Must hold a valid LPN license to practice in North Carolina (issued by the NC Board of Nursing).
- Skills and Training:
- Demonstrated ability to organize and manage weekly schedules.
- Strong problem-solving skills and a client-focused approach to care.
- Competence in using office software and maintaining accurate records.
- Familiarity with evidence-based practices in mental health care.
- Physical Requirements:
- Ability to perform light work (e.g., no lifting over 20 pounds).
- Must be free from all illicit substance use.
