Community Based Nurse (RN) - Assertive Community Treatment

Key Responsibilities

• Medical and Clinical Oversight:

  • Monitor and document all medical and clinical aspects of client care.
  • Administer psychopharmacologic and medical services within the scope of licensure.
  • Provide symptom assessment, management, and crisis intervention.

• Service Coordination:

  • Collaborate with the ACT team to track clinical status and treatment response.
  • Consult with primary healthcare providers and coordinate care with state and community inpatient facilities.
  • Develop transition plans for clients moving in or out of ACT services.

• Direct Client Care:

  • Deliver individual counseling, psychotherapy, and substance abuse treatment.
  • Support clients with activities of daily living, work-related services, and interpersonal relationship-building.
  • Educate and provide consultation to clients’ families and natural supports.

• Documentation and Planning:

  • Maintain accurate records, including treatment and goal plans, activity notes, and service documentation.
  • Identify and implement evidence-based or emerging best practices therapies for client care.

• Team Collaboration:

  • Participate in rotating crisis phone/on-call duties with team members.
  • Work closely with the ACT team to ensure seamless coordination and delivery of care.

Qualifications

• Licensure: Must hold an active Registered Nurse (RN) license in North Carolina, issued by the NC Board of Nursing.
• Experience: Prior experience in mental health or community-based care is preferred.
• Skills:
  • Strong organizational and communication skills.
  • Ability to work collaboratively in a multidisciplinary team setting.
