Carolina SeniorCare is a PACE (Program of All-Inclusive Care for the Elderly) based in New Bern, North Carolina.

PACE programs operate in 33 states with the mission to provide comprehensive, personalized healthcare services including primary care to elderly people 55 who are at risk for needing placement in a skilled nursing facility. A primary goal of PACE is to keep these vulnerable individuals in the community setting so long as they can safely be cared for in the community. This goal allows a focus more on prevention of disease than is typically done in a traditional primary care setting where treatment of disease is typically the focus.

The provider (PA or NP) served as PCP under the guidance of Medical Director and function as part of the care team.

Physician Assistant or Nurse Practitioner – Full Time

OVERVIEW

The provider job is a clinic-based Monday-Friday 8:00 am-4:30 pm position with no after-hours, weekend, or major holiday work or call required.

QUALIFICATIONS:

1. Medical clearance for communicable diseases and up-to-date immunizations before having direct patient contact.

1. Must have a valid driver’s license, proof of insurance and have means of transportation. Must be able to meet Carolina SeniorCare's auto insurance carrier's standards for coverage.

2. Current Physician Assistant license in North Carolina or Nurse Practitioner license in North Carolina (for Nurse Practitioner, must have current RN License)

3. Education: Bachelor’s Degree from an accredited university. Graduate of certified or accredited Physician Assistant Program/Nurse Practitioner program. Current Cardio
Pulmonary Resuscitation (CPR) certification required (will certify on site).

4. Experience: A minimum of one year’s experience in working with the frail or elderly population required.

ESSENTIAL FUNCTIONS:
1. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on plan of care planning decisions and coordination for 24 hour care delivery.

2. Performs in-person comprehensive health histories and physical exams on new Carolina SeniorCare participants. Coordinates with Interdisciplinary Team to develop a comprehensive plan of care for each participant.

3. Integrates the primary care treatment plan into the overall plan of care developed by the Interdisciplinary Team. Interacts with team members to meet emergent and acute need of participants. Participates in discharge planning for acute and long-term placement.

4. Following consultation with the Primary Care Physician, refers participants to medical specialist as indicated.

5. As requested and directed by the Primary Care Physician, manages care of participants in the nursing home: Provides regular visits as dictated by nursing home standards and participant need. Performs telephone contact with nursing home staff as required.

6. Provides relevant participant and caregiver education. Educates and trains participant and caregivers in appropriate care and interventions.

7. Orders, interprets, and follows up on lab tests and results. Performs selected procedures and documents orally and in writing. Initiates emergency measures as indicated.

8. Recognizes abnormalities that justify immediate referral for intervention by others.

9. Assesses, initiates, and coordinates participant care on a daily basis under the supervision of the Primary Care Physician.

10. Acts within the scope of his or her authority to practice

11. Performs other duties as required or requested.
