Under indirect supervision, provides, oversees, and / or administers a range of psychosocial evaluation / assessment, diagnostic, supportive counseling, therapy, and crisis intervention services in a clinical or medical in the consumer’s natural environment which requires a high degree of independent decision-making, intentional intervention, and empathy. Aspects of this job may require clinical assessment, evaluation, and intervention (individual, family, and / or group) of Medicaid Eligible consumers in an outpatient clinical setting or in the consumer’s natural environment.
Key Responsibilities : Conduct comprehensive assessments and evaluations of consumers' mental health and emotional well-being. Develop and implement individualized treatment plans in collaboration with consumers and their support systems. Maintain accurate and up-to-date consumer records, including progress notes and service plans. Exhibit cultural responsiveness in communications and interactions with consumers, MCOs, providers, and other stakeholders to ensure services are provided in an equitable and accessible manner to the communities served.
Requirements :
Qualifications : Master’s degree in social work, or counseling required, and licensed or provisionally licensed in respective field. Active North Carolina licensures such as LCSW, LCMHC, LPC, LMFT, or equivalent will be given priority consideration.
Strong knowledge of culturally responsive evidence-based therapeutic modalities and best practices. Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations and community stakeholders.
Ability to work independently and as part of a multidisciplinary team. Proficiency in electronic health record (EHR) systems and basic computer applications. Ability to read, analyze, and comprehend clinical records, human service periodicals, professional journals, technical procedures, or government regulations.
Experience : System of Care. Strong clinical assessment and therapeutic relationship experience skills with all ages. Experienced in Cognitive Behavioral Therapy, Trauma-informed Care, person-centered / survivor-centered approach, experiential therapeutic modalities.
Certificates, License, Registration : Must LCSWA, LCMHPA under board supervision, or LCSW, LCMHP, LPC, LMFT, medical professional with psychiatric certification. Comprehensive Clinical Assessment certification, Trauma-Informed Care Certification. EMDR certification a plus!
