We are seeking a Qualified Professional with experience in CST, TCL housing, and other housing programs to join our dedicated team.
Responsibilities
- Complete required documentation, including:
- DLA-20
- Service Authorization Request
- Person-Centered Plan (PCP)
- Coordinate, access, and provide Community support services and other relevant support services in accordance with wraparound philosophy
- Work with a Community Support Team (CST) that operates 24/7/365, ensuring continuous support for individuals with mental health and substance use disorders
- Assist individuals in community integration and ensure service intensity is adjusted as needed
- Maintain a beneficiary-to-staff ratio of 12:1, with a maximum team caseload of 48 individuals

Knowledge & Expertise
- Community Support Team policies and procedures
- Clinical diagnosis and treatment approaches
- MH/DD/SAS service network and principles
- State services and definitions in MH/DD/SAS
- Diagnostic and Statistical Manual (DSM)
- Basic office procedures (record-keeping, filing systems)
- Strong proficiency in English language, grammar, and communication
Key Duties
- Provide psychoeducation per the PCP
- Assist in crisis interventions
- Support the Team Lead in behavioral and substance use disorder treatment
- Develop relapse prevention and disease management strategies
- Participate in PCP development, implementation, and revisions
- Monitor and document client progress and effectiveness of interventions
- Conduct functional needs assessments
- Ensure linkage and referrals to appropriate services
- Assist beneficiaries in housing searches, financial aid, and household setup
- Engage landlords, draft reasonable accommodation letters, and address housing crises
- Help beneficiaries develop daily living skills to stabilize housing
- Provide ongoing monitoring, follow-ups, and crisis mitigation
Work Schedule & Requirements
- Flexible hours based on consumer needs
- Some travel required, including potential transportation of consumers/families
Minimum Education, Training & Experience
- Bachelor’s degree with 4 years of experience in a human services-related field under a QP
- OR 30 semester hours in a human services field (e.g., psychology, social work, counseling, nursing, education, occupational therapy, recreational therapy) with at least 1 year of experience serving the target population
