What's my primary role?
- Assist the Director of Social Services in facilitating and leading care conference meetings.
- Assist the Director of Social Services to develop with the Interdisciplinary Care Team (IDT) and update as needed a plan of care to address identified patient/resident needs as well as implementation of same.
- Assist with ABN/NOMNC notices for Medicare beneficiaries.
- Assist with Medicaid renewals and follow up.
- Assist the Director of Social Services to provide education to residents and staff regarding Resident Rights and Advanced Directives and advanced care planning to meet regulatory and/or center guidelines.
- Assist the Director of Social Services to develop, organize and coordinate available center and community resources to provide the highest quality social services program, while meeting the needs and interests of the residents.
- Assist the Director of Social Services with the completion of resident assessments, MDS section completion, CAA completion, and care planning duties within required timelines after receiving appropriate training.
- Maintains a positive working relationship with staff, families and all persons or agencies concerned with the well-being of the resident.
- Promote and demonstrate a culture of Service Excellence
Qualifications:
- Bachelor's degree in social work or human services (such as sociology, gerontology, special education, rehabilitation counseling, and psychology) and one year supervised social work experience in a healthcare setting working directly with individuals.
- Trained in recognizing and assessing the emotional and social, and medical social needs of residents; and is knowledgeable of community agencies and resources available to meet those needs throughout care transitions.
- Knowledge of third-party payer requirements for skilled patients and long-term residents.
- Knowledge of state regulations and MDS assessments for SNFs.
- Strong customer service and leadership skills.
- Strong interpersonal and communication skills; both written and verbal.
- Experience using Microsoft office programs (Word, Excel, Adobe and PowerPoint).
- Must be flexible, detail-oriented, ability to multi-task, meet deadlines and be able to make effective decisions and work as part of a service excellence team.
