Position Overview:
The Admission Nurse/Manager (LPN/RN) will oversee the admissions process for new residents, ensuring a smooth transition into our facility. This role involves coordinating with interdisciplinary teams, communicating with families, and managing admission-related documentation. The Admission Nurse/Manager will work the 3 pm - 11 pm shift and will play a key role in maintaining quality care standards.
Key Responsibilities:
- Coordinate and manage the admission process for new residents.
- Complete admission assessments and develop initial care plans.
- Communicate effectively with residents, families, and healthcare teams.
- Maintain accurate and thorough admission documentation.
- Monitor the progress of new residents during the transition period.
- Ensure compliance with state and federal regulations.
- Provide leadership and support to nursing staff during the admission process.
- Manage and resolve any admission-related issues.
- Serve as a resource for clinical and administrative support during the shift.
Qualifications:
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a current, valid license in Michigan.
- Previous experience in admissions, skilled nursing, or long-term care preferred.
- Strong organizational and communication skills.
- Ability to work independently and manage multiple tasks.
- Additional duties as assigned.
