What are the responsibilities and job description for the Service and Support Administrator (SSA) position at Carroll County Board of Developmental Disabilities?
Is responsible for the development and implementation of individual plans to assess, review, and meet needs of individuals with developmental disabilities. Plans and coordinates establishment of reasonable goals and objectives. Provides training and assistance to service staff as needed. Makes financial recommendations regarding residential/or adult services based on individual budgets and available funds.
Maintains regular contact with individuals and families/advocates/representatives and/or guardians of assigned caseload. Completes initial packets, redetermination packets and all other related applications for services for individuals as assigned and directed in a timely fashion.
Works cooperatively and communicates effectively with county board staff, MEORC staff, consumers of service, their families and advocates, providers, state and local agencies and others in a facilitative and proactive manner. Is responsible for coordination of the assigned caseload for assisting with the caseloads of other SSA’s when assigned to promote effective day to day operations of the Carroll County Board of DD services programs including but limited to Adult Services, Medicaid Waivers and Supported Living in accordance with policy and applicable law.
Monitors individuals progress in individual plans, conducts periodic program reviews and evaluations to ensure quality of services and adherence to established legal and certification standards, and coordinates procedures for progress reporting as related to elements of developed plans.
Provides liaison with parents, service agencies, community organizations, and other involved parties (e.g., parent groups) relative to planning, implementation, and progress; coordinates community relations efforts and participates in organized committees. Maintains familiarity with current & proposed legislation//rules affecting services via internet and other means. Participates in accreditation process with various agencies. Maintains confidentiality of program and consumer information and records. Works cooperatively with local Non Profit Housing Board, Gateway.
