Employment Type: Part Time (< 20 hours/week) Could be contract/per diem
Job Summary:
The Master's Social Worker (MSW) in the dialysis unit plays a crucial role in providing psychosocial support, counseling, and resource coordination for patients undergoing dialysis treatment. The MSW works collaboratively with an interdisciplinary team, including nephrologists, nurses, dietitians, and administrative staff, to enhance patients' quality of life, ensure treatment adherence, and address social, emotional, and financial barriers to care.
Key Responsibilities:
Patient Assessment & Counseling:
Conduct comprehensive psychosocial assessments for new and existing dialysis patients.
Provide individual and family counseling to address emotional distress, depression, anxiety, and coping mechanisms related to chronic kidney disease (CKD) and dialysis.
Develop and implement individualized care plans based on patient needs, goals, and available resources.
Assist patients with crisis intervention, grief counseling, and end-of-life discussions when necessary.
Care Coordination & Advocacy:
Collaborate with the interdisciplinary care team to develop holistic treatment plans.
Assist patients with navigating complex healthcare systems, including referrals to specialists, home health services, palliative care, and transplant programs.
Advocate for patient rights and access to appropriate healthcare services, insurance coverage, and financial assistance programs.
Resource Connection & Social Determinants of Health:
Provide education on financial assistance programs such as Medicaid, Medicare, Social Security Disability (SSD), and pharmaceutical assistance programs.
Connect patients with community resources, including transportation services, housing assistance, food programs, and mental health services.
Assist patients with employment concerns, vocational rehabilitation, and access to vocational support services.
Patient & Family Education:
Educate patients and families about CKD, dialysis treatment options, and lifestyle modifications to improve quality of life.
Conduct support groups or educational workshops to help patients and caregivers manage stress, treatment adherence, and long-term planning.
Documentation & Compliance:
Maintain accurate, timely, and confidential documentation of patient interactions by federal, state, and organizational policies.
Ensure compliance with regulations set forth by the Centers for Medicare & Medicaid Services (CMS), the State of Louisiana, End-Stage Renal Disease (ESRD) networks, and other governing bodies.
Participate in quality improvement initiatives, audits, and data collection for program evaluation.
Professional Development & Team Collaboration:
Attend interdisciplinary team meetings and contribute to patient care discussions.
Stay updated on best practices, policies, and emerging trends in nephrology social work and ESRD care.
Participate in staff training and mentorship of new employees or social work interns.
Qualifications & Requirements:
Education: Master's degree in Social Work (MSW) from an accredited institution.
Licensure/Certification: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) as required by state regulations.
Experience: At least 1 year of experience in dialysis social work, nephrology, or chronic disease management.
Skills:
Strong interpersonal, counseling, and crisis intervention skills.
Knowledge of ESRD, dialysis treatment, and healthcare systems.
Ability to work effectively with an interdisciplinary team.
Proficiency in electronic medical records (EMR) documentation.
Work Environment & Physical Demands:
Work is performed in an outpatient dialysis clinic, hospital, or home dialysis setting.
May require occasional travel for home visits or community outreach.
Requires sitting, standing, walking, and occasional lifting of up to 20 lbs.
