Job Summary

The Facility Administrator plans, coordinates and manages all services and employees of facility and is responsible for the overall direction, coordination and evaluation of all care and services provided to the Elders of the facility. The Facility Administrator oversees and maintains quality care, service and culture that is consistent with and exceeds organizational, state and federal regulatory standards as directed by the Plainview team. The Facility Administrator has the ability to communicate effectively and advocate for Elder centered / directed care to team members, families, physicians and the at large community.

Essential Functions

  • General

Works minimum of 40 hours / week. Normal working hours 0800-1630 Monday through Friday, but will participate in weekend leadership rotation, including evenings, nights, holidays, and weekends depending on needs of contract facility operations.

  • Will always dress and conduct him / herself in a professional manner in accordance with all policies and procedures.
  • Will maintain a professional, neat, and organized office, presentable at any moments' notice to the public as a meeting place.
  • Will maintain, respectful, cordial, and professional relations with all employees, residents, families, vendors, and the general public at all times.
  • Understands and orchestrates team based and individual action plans in coordination with the Plainview team, and each department head of the facility.
  • Understands and manages the budget and financial outcomes of the facility.
  • Understands all Federal Regulations (State Operations Manual) and State Department on Aging Regulations, all Life Safety Code Regulations, and ensures all said regulations are covered by a facility policy and / or procedure, and conversely, ensures all existing policies and procedures are compliant with said regulations.
  • Ensures that each employee working at the contract facility has a Job Description, has read it, understands it, and complies with it in full.
  • Employ a team of qualified individuals capable of serving as Department / Service area leaders to organize, manage and lead the operational day-to-day activities of the contract facility.
  • Is intimately familiar with all Federal Regulation (State Operations Manual) and State Department on Aging regulations and ensures that all regulations are covered by a policy and / or procedure, and that conversely all existing nursing policies and procedures are in accordance with said regulations in coordination with Director of Nursing, medical director and staff, drives the formulation of written policy and procedures for resident care and make those policies available to regulators as required by law.
  • Manages compliance with all policies and procedures of Plainview and Facility.
  • Manages compliance with all HR systems pertaining to recruitment, retention, hiring, disciplining and firing of employees.
  • Manages compliance with HIPAA.
  • Manages compliance with Worker's compensation policy.
  • Manages compliance with all other insurance policies.
  • Manages a safe-work program.
  • Manages incident reporting and investigation program and manages reporting of all incidents and accidents pertaining to the abuse, neglect and exploitation of elders to the State hotline.
  • Reports all risk management issues to Plainview Healthcare within 24 hours.
  • Notifies Plainview Healthcare within 24 hours of any notice from CMS and any state or Federal Agency or Attorney's office.
  • Manages relations with all 3rd party vendors.
  • Thoroughly understands and manages the entire budget, including census driven PPD formulas and administrative actions associated with it. Understands the agreed upon PPD's for RN staffing, LPN staffing, CMA / CNA staffing, RA staffing, Dietary staffing, Food, Nursing supplies, Pharmacy and Ancillary supplies and Overtime allowance. Reports outcomes of each pro-actively and retrospectively to the Regional Administrator on a monthly basis.
  • Communicates with Administrative team regarding human resource issues including compliance, Worker's compensation, performance and hiring / firing needs in accordance with organizational policy and procedure, and all federal and state laws and guidelines.
  • Understands, practices and communicates Elder directed / centered care on a daily basis. Holds all employees accountable to the principles and values of the Facility.
  • Assists with hiring and providing orientation / training and retention of a sufficient number of qualified staff to carry out all programs and services. Ensures employee performance meets or exceeds expectations and periodic performance appraisals are conducted on a timely basis. Holds department leaders accountable for completion of all of the above.
  • Oversees and helps develop education, in-services training and program development.
  • Oversees and conducts work necessary for business office management as required for accounts payable, accounts receivable and payroll in coordination with Plainview Healthcare.
  • Marketing and Outreach
  • Employs and manages the activities of the Marketing / Admissions Coordinator. Meet weekly with Marketing / Admissions Coordinator. If the size of facility does not allow for this FTE, then Administrator will serve in this capacity or delegate tasks to other existing facility FTE's and manage them accordingly.

  • Form a marketing action plan with the marketing team.
  • Ensure that census is communicated daily, including all referrals, admissions, discharges and transfers are reported to all other department / service area leaders and Plainview Healthcare on a daily basis.
  • Ensure that Facility maintains or exceeds budget census.
  • Business Office
  • Employs and Manages the activities of the Business Office Manager. Meet weekly with Business Office Manager. If size of facility does not allow for this FTE then Administrator will serve in this capacity or delegate tasks to existing FTE's and manage them accordingly.

  • In coordination with the Business Office Manager, meet monthly with the Business Office team to review outcomes, make decisions, and refine processes.
  • Form an action plan with the Business Office Manager.
  • Be familiar with and support the business office team with all processes for billing private pay, Medicaid, Medicare, and VA payment sources.
  • Be familiar with and support the Business Office team with all processes associated with accounts payable.
  • Be familiar with and support the Business Office team with all processes associated with payroll.
  • Be familiar with and support the Business Office team with all operation of phone system and receiving of guests.
  • Be familiar with and support the Business Office team with all operation of technology and software throughout the Facility. Ensure that needs are communicated to Plainview Healthcare members.
  • Facilities Management / Maintenance
  • Employ a qualified facilities manager / Director of Maintenance. Meet with facilities manager on a weekly basis.

  • In coordination with the facilities manager and Pelican Care Centers, form a facilities management / maintenance team.
  • Meet monthly with facilities management / maintenance team and form action plan for facilities maintenance and improvement projects.
  • Hold facilities manager and team accountable to agreed upon actions and projects.
  • Be familiar with the required Life Safety Code. Ensure Facilities manager is compliant with all required regulations, forms, reports, and checks.
  • Ensure that the Facility grounds and facilities are in excellent and presentable condition to the public at all times.
  • Report needs to Plainview Healthcare.
  • Social Services
  • Employ a qualified Licensed Social Worker full time, serving as Director of Social Services. Meet weekly with Director of Social Services.

  • In coordination with Director of Social Services, develop an action plan to streamline and improve quality of Social Services and The Contract Facility in accordance with Federal and state regulations.
  • Support the Director of Social Work in executing action plan.
  • Recreation / Activities
  • Employ a qualified Activities Professional in accordance with State and Federal regulations as Director of Recreation for the Facility. Meet weekly with Director of Recreation.

  • In coordination with Director of Recreation, develop an action plan to streamline and improve quality of Social Services and the Facility in accordance with Federal and state regulation, and the profession of Social Work.
  • Support the Director of Recreation in executing action plan.
  • Dietary and Nutrition
  • Employ a Certified Dietary Manager to serve as Food Service Director. Meet with Food Service Director on a weekly basis.

  • Develop an action plan in coordination with Food Service Director to ensure that the outcomes, policies and procedures of Dietary / Food services and Nutrition, meet all Federal (State Operations Manual) and State regulations, and that services are provided in accordance with resident's plan of care.
  • In coordination with Food Service Director, host a monthly food committee involving residents and staff to focus on increasing satisfaction.
  • Hold Food Service Director accountable to budgetary guidelines.
  • Nursing Services
  • Employ a qualified Director of Nursing. Meet with Director of Nursing on a weekly basis.

  • Develop an action plan in coordination with Director of Nursing to ensure that the outcomes, policies and procedures of Nursing services meet all Federal (State Operations Manual) and State regulations, and that services are provided in accordance with resident's plan of care.
  • Learn about CMS.gov 5 star rating system and manage process to make The Contract Facility a 5 star facility.
  • Manage survey outcomes and process to ensure no G deficiencies.
  • Hold Director of Nursing accountable to nursing outcomes and budget.
  • Other
  • Demonstrates the ability to set and establish priorities.

  • Manages time effectively.
  • Always provides proper notice in advance of absence.
  • As an exempt position works a minimum of 40 or more hours weekly.
  • Maintains work area in a neat and orderly manner.
  • Maintains a well-groomed professional appearance.
  • Drives the creation of, and ongoing activities of the Quality Improvement Committee (QAPI).
  • Serves as a positive role model to all other Facility employees, residents, and families.
  • Is familiar with and upholds the standards of conduct of the organization.
  • Believes in and upholds the values of Pelican Care Centers and the Facility.
  • Professional Requirements

  • Qualifications

  • The Administrator must have a Bachelor's degree from an accredited college and be currently licensed in the Facility's State without encumbrance, be responsible for maintaining professional licensure and CEU requirements, or have entered into a qualified Administrator in Training program with an accredited institution and qualified preceptor.
  • Knowledge, Skills, and Abilities

  • Well-versed in Federal Regulations (State Operations Manual) and the State Department on Aging Regulations, and ensures that all policies, procedures, and practices are in line with, and performed in accordance with those regulations.
  • Must be technically oriented, detail oriented and proficient in the use of computers and electronic health record systems.
  • Administrator for an Adult Care Home is a personal and professional responsibility. The Administrator by nature of that responsibility is on call 24 / 7, unless other arrangements for fulfilling ability to report to work quickly, have been made and agreed upon by Plainview Healthcare management.
  • Must be able to read and interpret financial statements, income statements (profit and loss) and balance sheet, and to hold others accountable to budgetary performance.
  • Strong organizational and interpersonal skills.
  • Ability to determine appropriate course of action in more complex situations.
  • Ability to work independently, exercise creativity, be attentive to detail, and maintain a positive attitude.
  • Ability to manage multiple and simultaneous responsibilities and to prioritize scheduling of work.
  • Ability to maintain confidentiality of all medical, financial, and legal information.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to handle difficult situations involving patients, physicians, or others in a professional manner.
  • Physical Requirements and Environmental Conditions

  • Exposure to general resident care environment .
  • Moderate exposure to temperature environment.
  • Exposure to pharmacological preparations, biological, and potential biohazard materials including blood and other bodily fluids and medical sharp objects.
  • May be required to work extended hours.
  • Frequent interruptions occur at work.
  • May be exposed to miscellaneous office and cleaning chemicals
