Carrus Lakeside Hospital offers primary care, urgent care, and therapy services to patients in Bristow and the neighboring communities of Oklahoma.
ESSENTIAL FUNCTIONS:
Under the supervision of a Physical Therapist, provides physical therapy services, reassessing the patient’s status during each treatment to ensure quality of care as evidenced in the clinical record.
Develops, discusses, and communicates a plan of care for each patient, in collaboration with each patient/family/significant other to address identified needs.
Maintains up-to-date and accurate documentation of therapy services provided to ensure the integration of information for use by the healthcare team.
Reports to work in a timely manner and adheres to policies, procedures, and processes.
Performs other job duties as needed and/or as assigned.
QUALIFICATIONS:
Requires basic computer skills.
Long term acute care and rehabilitation experience is preferred.
Must be able to effectively communicate in English via verbal and written means.
LICENSE AND CERTIFICATIONS:
Requires a current state Physical Therapist Assistant license in accordance with the Board of Physical Therapy.
BLS certification is required upon hire.
