About the Role - The Case Management Manager assists the Director of Integrated Care Management (ICM) in leadership, coordination, and supervision of the case management department. This position is responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources while respecting patient’s right to self-determination. The Manager has the overall responsibility for ensuring care is provided at the right level, right time and with the right resources. Primary duties of this position include supervision of day-to-day activities, financial accountability, authority to hire, and providing coaching for performance improvement to staff as needed in collaboration with the Director.
Qualifications
Required:
- Minimum two (2) years of acute care hospital patient care experience
- Two (2) years progressive leadership experience
- Current unrestricted Register Nurse license in the State of Nevada
- Understanding of Federal Regulations related to discharge planning and Hospital utilization management
- Organizational skills, excellent verbal & written communication skills, ability to lead, coordinate diverse group in fast paced environment, critical thinking, problem solving and negotiation skills
Preferred:
- Bachelor of Science in Nursing
- Accredited Case Manager (ACM)
- Minimum two (2) years of experience as acute care case manager
