Position Summary
Our Social Service Director establishes and promotes the vision and direction of the Department of Social Services consistent with state and federal mandates, and the County's strategic plan as approved by the Board of Supervisors; establishes and implements departmental goals and objectives; establishes appropriate administrative policies, services, structures, controls, and reporting systems for the efficient and effective performance of social service, income maintenance, employment training, and related functions; determines types and levels of services to be provided and standards by which delivery will be evaluated.
Responsibilities
- Supervise the social services department in the day-to-day operations ensuring to set job assignments and set priorities.
- Develop goals and objectives for the department; motivate and manage Care Team Member s to accomplish the same.
- Ensure new Care Team Member s are effectively oriented; ensure an ongoing educational program is in place as it relates to resident abuse, resident rights and other social services programs .
- Develop positive relationships on behalf of the community with regulatory agencies, residents, and families including participating during family orientation and family council.
- Assist in preparing budget for the department; administer approved budget.
- Implement policies / procedures with follow-up and supervision of Care Team Member s to ensure compliance.
- Provide direct psychosocial intervention .
- Perform resident assessments at admission, upon condition change, and / or annually.
- Create, review, and update care plan and progress notes.
- Provide direct psychosocial intervention.
- Coordinate resident visits with outside services, dental, optical, etc.
- Attend and document resident council meetings.
- Assist resident families in coping with skilled nursing placement, physical illness, and disabilities of the resident as well as the grieving process.
- Work with the resident, family, and other Care Team Member s to plan discharge.
- Conduct in-service programs to educate staff regarding psychoso cial issues and patient rights.
- Must demonstrate empathy, courteous, kind and professional workplace behavior and customer services to all residents, Care Team Member s, vendors, visitors and family members at all times.
- Perform other tasks as assigned within scope of practice .
