Description

Job Summary:  Pharmacists fill orders for medications, monitor patient medication therapies, and provide drug information. Pharmacists supervise and direct support personnel. In the absence of the director of pharmacy, a pharmacist may be required to assume the essential responsibilities and perform the duties of the director of pharmacy.


Essential Job Duties:   

1. Ability to work independently with minimal direct supervision.

2. Ability to work cooperatively with health-system and pharmacy staff.

3. Ability to handle frequent interruptions and adapt to changes in workload and work schedule.

4. Ability to set priorities makes critical decisions, and responds quickly to emergency requests.

5. Ability to exercise sound professional judgment.

6. Ability to communicate effectively (orally and in writing)/

7. Ability to meet the pharmaceutical care needs of neonatal, pediatric, adolescent, adult, geriatric, psychiatric, oncology, obstetric, rehabilitation, and skilled-nursing patients.

8. Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete patient medication profiles.

9. Prepares and dispenses medications, including sterile, chemotherapy, and parental nutrition preparation safely and accurately using appropriate techniques.

10. Issues controlled substances to patient care areas and maintains records as required by law.

11. Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and does, and therapeutic duplications.

12. Assists with pharmacokinetics consult service and drug dosing per organizational protocol.

13. Reviews/ interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed.

14. Reads, extracts, and interprets information in patient charts accurately.

15. Detects and reports suspected adverse drug reactions and medication variances accurately and in a timely manner.

16. Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational medication therapy selection.

17. Provides clinical consultation and clarification to practitioners. Recommends evidence-based medication therapy regiments and monitoring plans. Suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed.

18. Provides accurate, adequate, and timely drug information to the professional staff.

19. Provides drug information to patients and their families.

20. Documents all clinical activities and interventions accurately and completely.

21. Participates in the quality improvement and medication-use review activities of the department. Collects data; conducts quality monitors and inspections; and maintains logs, records, and other documentation as assigned.

Requirements

 Licenses or Education: 

1. Graduate of an ACPE-accredited school of Pharmacy with a BS or Doctor of Pharmacy degree.

2. Active pharmacist license awarded by the State of Maine.

3. Health-system experience preferred, but not required.
