Essential Job Duties:
- Provide professional social work services to inpatients and manage referrals from outpatient settings.
- Facilitate adjustment of patients to the impact of illness and promote maximum benefit from healthcare services.
- Evaluate case situations and present information to the Interdisciplinary Team.
- Ensure delivery of care that respects individual rights and preferences.
- Maintain high standards of patient care using professional specialty standards of care and practice.
- Prioritize a safe environment for all patients.
- Evaluate patients with psychiatric and/or mental disorders and identify strategies for management while in the hospital.
- Make appropriate referrals and follow-up care.
- Provide case management functions to new mothers under 18 and their infants.
- Understand the needs of the elderly and provide discharge planning to nursing homes, boarding homes, and rehabilitation services.
- Coordinate patient care activities with other departments contributing to patient care.
- Communicate effectively and have good organizational skills.
- Display understanding of psychodynamics of all age groups and treatment strategies for common health problems.
- Ensure shared accountability for professional practice.
- Confers with other departments and management staff before instituting new policies and procedures.
- Ensure own professional compliance with applicable federal, state, and local regulatory agencies and with professional licensure.
- Maintain regular communications with medical staff physicians in the care and management of their patients.
- Organize interdisciplinary team meetings for patients when appropriate.
- Provide one-on-one counseling or group counseling as needed.
- Help patients modify attitudes and patterns of behavior by increasing self-understanding.
- Provide on-the-spot crisis management for patients who feel out of control behavior.
- Maintain availability to staff for mental debriefing with difficult cases.
- Keep abreast of new techniques and counseling strategies.
- Maintain current knowledge about variable resources in the community.
- Set priorities on current caseload and services.
- Respond timely for crisis calls when in hospital.
- Participate in department meetings to solve problems and improve processes.
- Be knowledgeable of department policies and procedures and participate in their updates.
- Develop appropriate policies and procedures related to social services.
- Participate in the orientation of new staff members.
- Help identify patient/family related ethical issues with referral to the Ethics Committee.
- Participate on hospital and medical staff committees when requested.
- Collaborate effectively with all members of the healthcare team.
- Act as a liaison with AMHC for Community Meetings as needed.
- Record pertinent social data and information on patients being managed.
