Hospice Care Expertise
We are seeking an experienced and skilled Hospice Care Expert to join our team at Casa de la Luz Hospice. The successful candidate will have a strong background in hospice care, with experience in managing complex symptoms and supportive care needs.
The ideal candidate will have a minimum of five years of nursing experience, with current certification as a Nurse Practitioner or clinical Nurse Specialist. Current driver's license and vehicle insurance are also required.
As a Hospice Care Expert, you will be responsible for making face-to-face encounter visits within prescribed time limits to qualifying hospice patients, reporting clinical findings to the Hospice Medical Director, and collaborating with other physicians as needed.
