About the role

Casa Esperanza seeks a dynamic, energetic individual who is passionate about delivering recovery and wellness-oriented outreach to people living with addiction, mental health and cooccurring disorders. The Casa Community Support Program (CSP) addresses the health-related social needs of individuals to help them thrive and live successfully in the community. CSP Recovery Coaching delivers support to individuals from peers with similar experiences to gain hope, explore recovery, and achieve life goals. It is a non-clinical and non-medical service; however, the program works closely with the Multi-Disciplinary Team to provide support for individuals as part of our integrated care model which includes primary care, mental health, and addiction treatment.

Under the direction of the Associate Director for Community Support Programs, the Coordinator will be responsible for working with referring organizations / agencies to transition clients into recovery coaching, which aims to support to clients in their efforts to access and remain engaged in substance use and mental health services. The Coordinator will work in collaboration with other programs in making referrals to local treatment providers and help clients to navigate the mental healthcare and drug / alcohol abuse recovery systems, as these are critical to their stability and behavioral health engagement. They will provide supervision and oversight to Recovery Coaches, in collaboration with the multidisciplinary team, working together to deliver integrated care under the CasaCare model. As part of the Casa Esperanza, Inc. team, they will play a lead role in providing services in a fully integrated, holistic, and collaborative manner using a team-treatment approach, to ensure the coordination of all client services, both across Casa programs as well as with external providers.

The Recovery Coach Services Coordinator must be able to initiate and complete work independently to fulfill the responsibilities of the job and work collaboratively with all members of the care team. While there may be opportunities for occasional work from home, this is primarily an in-person, community-based role that requires travel, providing transportation for patients, and in office time.

What you'll do

Service Delivery :

  • Deliver services in 1) linguistically appropriate and culturally competent manner that embraces the diversity of people's identities including racial, ethnic, gender / gender identity, sex, sexual orientation, and disability including intellectual disability, and 2) an integrated manner that meets the needs of individuals with co-occurring substance use disorder and mental health conditions, honoring their chosen pathways to recovery and wellness.
  • If the client comes from a 24-hour behavioral health level of care, including inpatient and diversionary providers, or directly from incarceration, the Coordinator will participate, as appropriate, in member discharge / transition planning at the referring provider.
  • Manage referral process within Recovery Coaching services, including reviewing assessments and screenings to determine eligibility.
  • Work with Peer Recovery Coaches to establish and coordinate a range of positive recreational, educational, cultural and community activities for patients and families, both virtually and in-person when possible.
  • Develop, schedule and facilitate support groups, virtually and in-person, when possible.
  • Ensure participants, their family members, and community providers are provided with information regarding substance abuse, mental health, HIV, and other health and wellness education through individual and group sessions.
  • Provide oversight to Recovery Coaches in facilitating peer support groups on campus,; assist clients in developing Wellness Plans, and access substance abuse and mental health treatment.
  • Willingly shares personal recovery experiences, demonstrates and models effective coping techniques and self-help strategies.
  • Participate in development and documentation of a peer leadership development model that supports the growth and sustainability of Recovery Capital for the Latine Community
  • Working with the Reentry Services Coordinator, the Supportive Housing Coordinator, and other Casa staff, provide comprehensive care coordination across Casa programs and externally across the network of services.
  • Work closely with Multi-Disciplinary Team (MDT) on all aspects of care planning, including assessments, transition and discharge planning to engage, retain and monitor clients in care.
  • Work closely with the Patient Access and Practice Management teams to troubleshoot enrollment, authorization, and reauthorization for clients engaging in Recovery Coaching services, as needed.

Program Planning & Partnerships

  • Coordinate and conduct regular outreach to service providers, treatment providers, peer support organizations and groups, and other community-based organizations
  • Collaborate with the Senior Associate Director of Recovery Support Services and Outreach Team to identify places for outreach and initiation of community partnerships.
  • Represent the agency by providing education and participating in community events, MOAR meetings to connect Casa Esperanza's Latine recovery community with CODs to the statewide addiction recovery community.
  • Initiate new partnerships with other organizations and community groups that broaden the range of activities, opportunities and available resources and build connection between treatment and recovery support services, particularly resources for Spanish speakers.
  • Maintain up-to-date lists of and contacts with SUD and mental health treatment, recovery support services, supported employment and / or vocational and education services, housing supports, and HIV services both at Casa Esperanza and at other agencies in the community.
  • Coach and support team members to leverage contacts and organizational linkages to support client access to care.
  • Advocate for Peer Recovery services, wrap-around supports and community care in accordance with Casa policies.
  • Work with the Outreach Team and Peer Recovery Coaches to develop and implement advocacy and outreach presentations to Latino-serving organizations, community groups, and events
  • Administration & Supervision

  • Monitor Case Manager assignments to ensure appropriate caseloads.
  • Establish and maintain efficient work flow to ensure client utilization meets program goals and objectives as specified in the Recovery Coach Productivity Agreement.
  • Ensure that wellness plans (or equivalent), 1 : 1 new client orientation and discharge planning are completed timely for each client.
  • Ensure that Peer Recovery Coaches submit all reporting as required by payors, regulators, grantors, and Casa's internal Practice Management and Quality teams, in a timely manner.
  • Complete periodic review of client records to ensure all client encounters and other data are documented as required.
  • Provide bi-weekly supervision to Peer Recovery Coaches, and an annual performance review for each staff member.
  • Ensure that Recovery Coaches are pursuing Recovery Coach Certification and / or maintaining their certifications.
  • Collaborate with Leadership to address any barriers to implementation or programs & services
  • Attend bi-weekly supervision with the Associate Director of Community Support Programs.
  • Attend all relevant case conferences and staff meetings including but not limited to Multi-Disciplinary Team, Community Support Program / Recovery Support Services, and Office Based Addiction Treatment.
  • Other duties as assigned.
  • Qualifications

  • Bilingual Spanish / English required.
  • Bachelor's degree or three years of relevant work experience, or lived experience, including previous experience with peer-based support services
  • Certified Peer Recovery Coach Supervisor, required or willing to obtain within one year of hire
  • Demonstrated supervisory and project management ability
  • Ability to understand and manage moral / ethical boundaries with clients / staff.
  • Knowledge of SUD and mental health treatment, the recovery process and resources, supported employment and / or vocational and education services, housing supports, and HIV services
  • Minimum two years' experience working with individuals with a history of or at risk for justice involvement, HIV, addiction, mental illness, trauma and violence
  • Able to work weekends and evenings, as needed
  • Familiar with social, health and cultural issues specific to the Latine population
  • Ability to work as a team member and independently
  • Understanding of harm reduction principles and techniques
  • Strong verbal and written communication skills
  • Strong organizational skills, interpersonal skills and crisis management skills
  • Be in good standing with their board of professional licensure, as applicable.
  • Computer skills, minimum Microsoft Word
  • Access to vehicle and a valid driver's license preferred.
  • Supervision : Will supervise Recovery Coaches and receive supervision from the Associate Director of Community Support Programs.

    Job Type : Full-time, Weekdays and every other Saturday
