Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. Duties
- Conduct thorough assessments of patients' medical, psychological, and social needs to develop individualized care plans.
- Collaborate with multidisciplinary teams to coordinate services and ensure continuity of care.
- Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.
- Provide education and support to patients and their families regarding treatment options, resources, and self-management strategies.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Facilitate communication between patients, families, healthcare providers, and community resources.
- Document all interactions, assessments, and care plans accurately in the electronic health record system.
- Advocate for patients' needs within the healthcare system to ensure they receive appropriate care.
Experience
- A valid Registered Nurse (RN) license is required.
- Experience in patient care settings such as medical-surgical, critical care, acute care, or emergency room (ER) is preferred.
- Familiarity with behavioral health practices is a plus.
- Strong skills in diagnostic evaluation and case management are essential.
- Excellent communication skills and the ability to work collaboratively within a team environment are necessary for success in this role.
- Knowledge of HIPAA regulations is required to ensure patient confidentiality.
