The LVN provides primary care to patients under the medical direction and supervision of the patients’ attending physicians and Executive Director of the facility, with an emphasis on assessment, illness prevention and health care management; assisting in modifying the treatment plan to meet the physical and psychosocial needs of the patient.
Responsibilities
- Consult with physician(s) concerning patient evaluation and assist the Director of Nursing in planning and developing the nursing services to be performed for the patient.
- Initiate requests for consultation or referral. Respond to requests from the patient, physician, or nursing staff.
- Examine patients and their records and charts to distinguish between normal and abnormal findings in order to recognize early stages of serious physical, emotional, or mental problems.
- Assist in developing and implementing appropriate plans of action to correct identified deficiencies.
- Assist in the scheduling of patient assessments, treatment plan development to ensure that such information is provided and transmitted on a timely basis.
- Monitor the facility’s QI, QM, and survey reports. Assist in developing plans of correction when potential problem areas are indicated.
- Maintain a current file/listing of patients by name and room number, emergency phone numbers of on-call personnel, department extensions, key personnel, etc.
- Provide assessment and diagnostic services to assigned patients.
- Perform an assessment evaluation and a complete in-depth physical examination using assessment techniques including observation, inspection, auscultation, percussion, and palpation. Perform optic and ophthalmic examinations.
- Record physical findings and formulate plans and prognosis based on patient’s condition. Report these to the attending physician.
- Order, interpret and evaluate diagnostic tests to identify and assess patient’s clinical problems and healthcare needs.
- Provide therapeutic services and confer with the Treatment Team in the development of the treatment plan.
- Give direct physical and psychological nursing care in emergencies, as well as in the presence of an illness or disability, in order to maintain life, provide comfort, reduce stress, and enhance the patient’s ability to cope.
- Coordinate the admission or discharge of a patient, as necessary.
- Work with other health agencies and family members in the physical and emotional transitions of admission, care, and discharge of the resident.
- Provide information to the patient and family members of prevention techniques and other ways to maintain or improve the general health and psychosocial status of the patient.
- Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand to meet the needs of patients.

Qualifications
- Licensed Vocational Nursing degree is required
- 1 year or a combination of field and CNA experience is required
- Must possess a current, unencumbered, active license to practice as an LVN in CA.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to health care facilities.
- Minimum of 3 years of sobriety is require
