What are the responsibilities and job description for the Behavioral Case Manager/Social Worker position at Casas Adobes Post Acute and Rehabilitation?
- Casas Adobes Post-Acute is looking for a great Behavioral Case Manager.
- Responsibilities include but are not limited to: case management, handling customer service complaints, self-reports, planning discharges, helping to grow census, interacting with physicians, planning and attending care conferences, etc.
- This is a full-time position. Experience with behavior patients is required.
- It is preferred that you have a degree in Social Work, Sociolology, Gerontology, Psychology, or other human services field.
