Bilingual Housing Case Manager

Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Bilingual Housing Case Manager. The individual works in Cascade AIDS Project’s Housing & Supportive Services Department to provide services to Spanish-speaking and English/non-Spanish speaking clients. The work includes providing home-based housing case management, goal planning, information and referral services, advocacy with and on behalf of clients, and eviction prevention for individuals and families living with HIV.

 

This is a Full-Time role working at our Davis St. location in Old Town of Portland, OR. The schedule for this role is Monday-Friday 9am-5pm.  

Starting pay is $22.77 $2.00 bilingual pay which is equivalent to just over $51,500.The Bilingual Housing Case Manager’s Pay increases with tenure and the top of the pay scale is $59,321. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

 

Who You Are

  • Fluent in Spanish and English.
  • Two years’ experience relevant to the position or bachelor’s degree in human/social services field (social work, public or community health, psychology) or related field  
  • At least two years of demonstrated success providing case management (or similar) services with knowledge of care coordination, client advocacy and community service navigation verified through previous employer references
  • Prior experience working with ethnic, racial, economic and sexually diverse populations and people experiencing homelessness, mental illness, and/or substance addiction
  • Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to: engaging in training workshops, team and department meetings and critical discussions for how CAP can better serve the communities we serve
  • Demonstrated computer proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet

 

 

What You’ll Do

  • Working with clients to develop housing plans, assisting clients in locating and securing affordable housing, mediation with landlords, completing forms
  • Housing placement, supportive case management, and eviction prevention with and on behalf of clients who are homeless or at risk of becoming homeless using a supportive strengths-based, harm reduction model that promotes client self-determination and independence
  • Work in collaboration with medical case managers, medical providers, or other providers involved in client care, to provide coordinated comprehensive care to PLWHA with a focus on housing stability 
  • Carry an active client panel of approximately 70 program eligible clients, assisting with eviction prevention, housing planning, advocacy, mediation, and information and referral
  • Complete comprehensive housing assessments, goal planning, linkage to services, and advocacy for and with clients
  • Make appropriate referrals to mental health, substance abuse, HIV prevention, or other community partners as needed to support client self sufficiency
