Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented OCEAN (Oregon Carceral Engagement & Access Network) Housing Navigator. The Navigator works in CAP’s Housing & Support Services department and in coordination with the statewide OCEAN team which includes OCEAN Navigators across the state and a Release Coordinator within Oregon Department of Corrections. The Navigator primarily works with clients who are being released, or have recently been released, from incarceration in the state of Oregon, and are releasing to the Portland metro area.
This is a Full-Time role working at our Davis St location in NW Portland. Travel throughout Portland’s 5 county areas is frequent. The schedule for this role is Monday-Friday 9-5, some weekends and evenings may be required.
The starting pay is $47,370.59. The OCEAN Housing Navigator’s pay increases with tenure and the top of the pay scale is $55,170.59. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.
Who You Are
- Bachelor’s degree in human/social services field (social work, public or community health, psychology) or related field and/or two years of experience relevant to the position
- Prior experience providing 1-2 years of successful case management (or similar) services
- Demonstrated computer proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet
- Excellent written and oral communication skills.
- Successful experience working with ethnic, racial, economic and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance addiction
- Demonstrated ability to effectively collaborate with community stakeholders
- Valid driver’s license and access to a reliable vehicle
What You’ll Do
- Carry an active client panel of approximately 15 program-eligible clients
- Provide supportive case management and housing navigation, eviction-prevention support, linkage to HIV care, and information and referral with and on behalf of clients who are transitioning from carceral systems using a supportive strengths-based model that promotes client self-determination and independence.
- Complete CAP Intakes and conduct comprehensive assessments and goal planning to understand housing needs, health status and potential barriers to housing for clients prior to release. Develop individualized service plans based on these assessments and available resources.
- Research, identify and provide suitable housing options including motel placement, permanent supportive housing, rapid rehousing programs or emergency shelters.
- Complete Ryan White Eligibility Determinations.
- Review clients’ medical histories, including HIV care, to assess their readiness for housing and identify appropriate housing options.
- In coordination with the client and client’s care team, identify immediate needs upon release including access to HIV medications, healthcare, legal support and transportation.
