Cascade AIDS Project is excited to announce that we are hiring a skilled and team-oriented Housing Case Manager for The Joyce. The Joyce is a Permanent Supportive Housing (PSH) program located in downtown Portland and CAP is partnered with Community Partners for Affordable Housing (CPAH), Native American Rehabilitation Association of the NW (NARA NW), Cascadia Health, and Pinehurst Property Management to provide subsidized housing and intensive supportive services to residents of the building.
This position works onsite with participants exiting long periods of homelessness and/or chronic homelessness and who may have co-occurring mental health and/or substance use and recovery needs who may require tenant education and other interventions for activities of concern.
This is a Full-Time role working at The Joyce location in Downtown Portland. The schedule for this role is Fully onsite Monday-Friday 9am-5pm.
The starting pay is $51,696.99. The Joyce Housing Case Manager’s Pay increases with tenure and the top of the pay scale is $59,496.99. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.
Who You Are
- Two years experience relevant to the position or bachelor’s degree in human/social services field (social work, public or community health, psychology) or related field
- At least two years of demonstrated success providing case management (or similar) services with knowledge of care coordination, client advocacy and community service navigation verified through previous employer references.
- Prior experience working with ethnic, racial, economic and sexually diverse populations and people experiencing homelessness, mental health, and/or substance use issues.
- Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to engaging in training workshops, team and department meetings and critical discussions for how CAP can better serve the communities we serve
- Demonstrated computer proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet
- Demonstrated excellent written and oral communication skills
What You’ll Do
- Provide housing placement, supportive case management, and eviction prevention with and on behalf of clients who are homeless or at risk of becoming homeless using a supportive strengths-based model that promotes client self-determination and independence
- Work in collaboration with medical case managers, medical providers, or other providers involved in client care, to provide coordinated comprehensive care to Trans and Non-Binary participants with a focus on housing stability
- Carry an active client panel of approximately 15 program eligible clients, assisting with eviction prevention, housing planning, advocacy, mediation, and information and referral
- Complete comprehensive housing assessments, goal planning, linkage to services, and advocacy for and with clients
- Makes appropriate referrals to mental health, substance abuse, HIV prevention, or other community partners as needed to support client self sufficiency
- Fax, mail or hand deliver a copy of each client’s housing plan to the medical case manager and provide regular updates as to the progress of each client towards achievement of their goals
