Job Summary:
Locations: O'ahu, Maui, Kaua'i, Hilo, and Kona

We are looking for a motivated individual that is self-driven and looking to run a case management agency. We are looking for nurses on O'ahu, Maui, Kaua'i, Hilo, and Kona. As a Registered Nurse Case Manager and leader, you will be responsible for providing comprehensive and coordinated care to patients, ensuring their needs are met throughout their healthcare journey, and overseeing a team of nurses. Your expertise in acute care and case management will be crucial in delivering high-quality patient care.
Responsibilities:
- Conduct comprehensive assessments of patients' healthcare needs and develop individualized care plans
- Collaborate with interdisciplinary healthcare team members to coordinate and manage patient care
- Monitor patients' progress and make necessary adjustments to the care plan as needed
- Provide education and support to patients and their families regarding their medical condition and treatment options
- Utilize medical records and diagnostic evaluations to inform decision-making and ensure accurate documentation
- Adhere to HIPAA regulations to protect patient confidentiality
- Collaborate with other healthcare providers, such as ER staff, ICU nurses, and critical care specialists, to ensure seamless transitions of care
Skills:
- Acute care experience in a hospital or clinical setting
- Proficient in case management principles and practices
- Strong knowledge of medical terminology and diagnostic evaluation processes
- Familiarity with critical care procedures and protocols
- Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare team members
