What are the responsibilities and job description for the Guardian position at Casey Ball Supports Coordination, LLC?
This position is a work from home position, that requires face to face monthly visits with wards in Cambria, Somerset, Blair, Huntington , Centre counties.
MUST HAVE BACHELOR'S DEGREE IN RELATED FIELD AND 1 YEAR EXPERIENCE.
SPECIFIC DUTIES
- Meets wards monthly in their home or in the placement
- Ensures they are receiving appropriate medical care, attends care conference, oversees that their needs are being met
- Updates required paperwork as needed for the medical assistance application
- Consults with supervisor regarding cases, care plans, and assessments, especially any problems or safety concerns.
- Accrues billable time at a rate set by supervisor.
- Maintains daily log documenting billable and non-billable time
- Assists participants with placement in a nursing facility.
- Completes daily and monthly reports as required.
- Provides Personal advocacy for participants to assist in resolving problems.
- Attends trainings as assigned
- Performs related work as required.
- REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
- Bachelor's Degree in Related Field
- Microsoft Office
- Social casework principles, practices and methodologies
- Individual, group, and family behavior and ways of working effectively with the elderly and disabled.
- Current economic, social and health problems of the elderly and disabled.
