Job Summary
The Health Navigator is a critical member of the healthcare team, responsible
for connecting individuals and families with the resources and services they

programs, and addressing barriers to care. The Health Navigator will work
closely with healthcare providers, social service organizations, and community
resources to coordinate services, ensure continuity of care, and empower
clients to make informed decisions about their health. Strong communication,
problem-solving, and advocacy skills are essential for this role, which aims to
improve health outcomes and enhance access to care for diverse populations.
Duties
- Client Support and Advocacy
- Assist clients in understanding their healthcare needs, treatment
- options, and available resources.
- Provide education on health insurance options, including
- enrollment and eligibility requirements.
- Advocate on behalf of clients to address barriers to care, including
- financial, logistical, and social challenges.
- Care Coordination
- Schedule and coordinate medical appointments, follow-ups, and
- referrals for clients.
- Collaborate with healthcare providers and social service
- organizations to ensure continuity of care.
- Monitor client progress and address any issues that arise during
- their care journey.
- Resource Connection
- Identify and connect clients with community resources such as
- food assistance, transportation, housing, and mental health
- services.
- Provide clients with information about preventive care, wellness
- programs, and lifestyle changes.
- Health Education
- Deliver one-on-one or group education sessions to clients on topics
- such as chronic disease management, preventive health, and
- navigating the healthcare system.
- Assist clients in understanding medical terminology, treatment
- plans, and health-related documents.
- Documentation and Reporting
- Maintain accurate and confidential client records in accordance
- with organizational policies and HIPAA regulations.
- Document client interactions, progress, and outcomes in the
- designated systems.
- Prepare reports on program performance, client outcomes, and
- identified gaps in services.
- Community Engagement
- Participate in community events, health fairs, and outreach
- programs to promote health services and increase community
- awareness.
- Build relationships with community partners and stakeholders to
- enhance the network of available resources.
- Program Development and Improvement
- Provide input on program development and identify opportunities
- to improve services and resources for clients.
- Stay informed about changes in healthcare policies, resources, and
- best practices to enhance service delivery.
- Emergency Preparedness
- Support public health emergency response efforts by assisting with
- resource distribution and client outreach as needed.
Experience
- Preferred Qualifications:
- Current or ability to pursue Community Health Worker Certification
- Associate’s degree preferred.
- Experience with public health programs.
- Minimum Qualifications:
- Proficiency in English (speaking, reading, and writing).
- Strong computer skills.
- Successful completion of:
- Medical evaluation upon hire.
- Immunization review upon hire.
- Drug screening upon hire.
- Background and Central Registry checks.
- Possession of a valid Wyoming driver’s license.

Pay: $47,000.00 - $58,630.00 per year
