We are looking for aFull-Time Physical Therapistto join our staff atHaven Homes, a senior care campus in Maple Plain, MN! We offer a competitive wage and benefits as well as a$5,000 bonus paid out over 1 year or tuition reimbursement. New grads are welcome to apply!

Physical Therapist Qualifications:
- Must be a graduate from an Accredited Physical Therapy Program and have a bachelor’s degree
- Must possess a current accredited MN Physical Therapist License
- Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care
- Develop care plans and discharge planning strategies for optimal patient recovery
- Collaborate with healthcare professionals to ensure comprehensive and personalized treatment plans
- Demonstrate excellent communication skills to educate patients and their families on therapy techniques and home care practices
- Must be capable of performing the essential functions of the job, with or without reasonable accommodations
- Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required
