Classification: Support
Job Title: Special Education Transition Specialist
Job Grade: Grade 111
Job Goal: Provide support to students on IEPs needing transition based assessments, connections to Careers and College, self-advocacy skills, knowledge of community resources, and referrals to community and state agencies as well as work closely with Vocational Rehabilitation on the contract requirements under VR in working with students with disabilities.
Minimum Qualifications:
- High School diploma
- Knowledge of office procedures and ability to operate basic office machines
- Ability to add, subtract, multiply and divide in all unites of measure using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent and to draw and interpret bar graphs
- Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations
- Ability to read and comprehend simple instructions, short correspondence and memos. Ability to present information to parents, students and employees
- Ability to work cooperative with fellow employees, teachers, students, and parents
- Knowledge of IRS payroll rules and regulations
- Knowledge of federal and state tax regulations as applied to payroll
- Knowledge of and adheres to federal, state and county rules, regulations and CUSD policies and procedures
- Skill in reading, interpreting, understanding and applying relevant federal, state and CUSD rules, regulations, policies and procedures
- Strong computer and technical skills and ability to work with standard software products
- Knowledge of fiscal practices and procedures
- Knowledge of related Business Services functions
- Knowledge of file and record management
- Satisfactory criminal background check
Core Job Functions:
- Maintain regular and punctual attendance.
- Liaison between the student, employer and agency staff.
- Coordinates services for students geared towards transition.
- Provides individualized and group instruction in job readiness skills and self-determination skills.
- Assists the student with Vocational Rehabilitation eligibility determination process.
- Facilitates post-secondary school visits.
- Identifies and develops community career exploration and work experience programs pertaining to job placement.
- Initiates contact with potential employers and explains special education career exploration and work experience programs as they pertain to job placement.
- Assists with training students in the use of appropriate transportation to and from work sites.
- Works closely with student, employer, as needed for orientation to new job sites.
- Maintains and submits accurate records regarding career exploration, job placement and development of job skills.
- Communicates student’s progress to his/her IEP manager
- Provides transportation to and from job site when appropriate.
- Responsible for the organization of schedules for all student career exploration and work experience.
- Acts as school liaison between parents, business and appropriate agency.
- Participates in professional development to support the transitional needs of students.
