Job Responsibilities
- Comprehensive Patient Assessments – Evaluate patients’ physical, emotional, and home environments to develop personalized care plans.
- Medication Administration & Management – Administer medications, IV therapy, and injections while monitoring for side effects and effectiveness.
- Wound Care & Post-Surgical Management – Perform dressing changes, infection prevention, and wound monitoring to ensure proper healing.
- Chronic Disease Management – Support patients with diabetes, heart disease, COPD, and other conditions through education and symptom monitoring.
- Patient & Family Education – Teach proper self-care, medication use, and disease management to enhance independence and safety.
- Skilled Nursing Procedures – Provide care such as catheter maintenance, tube feedings, injections, and blood draws.
- Monitoring & Reporting Changes – Track vital signs, mobility, and cognitive function, reporting changes for timely intervention.
- Coordination of Care – Work with physicians, therapists, and aides to ensure seamless patient care and treatment adjustments.
- Preventative & Wellness Care – Educate patients on fall prevention, infection control, nutrition, and exercise for better health outcomes.
- Accurate & Timely Documentation – Maintain clear, compliant records of assessments, care plans, and patient progress.
- Home Safety Assessments – Identify fall risks and recommend home modifications to improve safety and accessibility.
Who We’re Looking For
- Licensed RN in Illinois.
- Experience in Home Health (preferred but not required – we provide training).
- Strong Communication & Organizational Skills – Ability to work independently and as part of a team.
