Our Crisis Stabilization Unit (CSU) provides short-term, intensive support for individuals experiencing acute mental health and substance use crises.
Position Overview:
We are seeking a Part-Time Case Manager to join our Crisis Stabilization Unit. This role is responsible for coordinating care, providing support, and linking individuals to community resources to ensure a smooth transition from crisis stabilization to ongoing care. The ideal candidate is a compassionate, detail-oriented professional with experience in behavioral health case management.
Key Responsibilities:
Provide case management services, including coordination with medical providers, therapists, and community resources.
Assist clients with discharge planning, ensuring a seamless transition to appropriate outpatient care, housing, and support services.
Advocate for clients and provide crisis intervention as needed.
Maintain accurate and timely documentation in accordance with agency and regulatory standards.
Collaborate with CSU staff, including nurses, clinicians, and peer support specialists, to ensure comprehensive care.
Educate clients and families on available resources and treatment options.
Attend team meetings and contribute to treatment planning discussions.
Qualifications:
Bachelor's degree in social work, psychology, counseling, or a related field, or 3 years' experience in Behavioral Health Care
Experience in mental health, crisis intervention, or case management preferred.
Strong understanding of behavioral health services and community resources. Excellent communication, problem-solving, and organizational skills.
Ability to work collaboratively in a fast-paced crisis environment.
Proficiency in electronic health record (EHR) systems and documentation.
