As a Nurse Practitioner with Catalytic Health Partners, you are helping high risk patients address their holistic healthcare needs as part of an integrated team. You will see 5 patients per day in their home setting focusing on their needs and providing them with your expertise and care.

Area Covered: South Tucson area and Green Valley/Sahuarita
Total Compensation:
Base pay range: $130,000 to $150,000 base on experience
Bonuses: You earn bonuses in May and November of each year with possibility of earning up to an additional 20% of base salary as bonuses in a year.
Employer Paid Insurance: Catalytic Health Partners pays for your Health Insurance, Dental Insurance and a Life Insurance policy, this is an $8000 value that does not come out of your paycheck.
What We'll Expect from You and What You'll be Doing:
·Spending your day seeing 5 patients per day Monday through Friday with an 8-hour day.
·Clearly explaining the benefits of our program and how it caters to their unique
healthcare needs.
·Working with community providers and facilities ensuring patients receive the care they
need.
·Spend time educating patients on their diagnosis, medications, healthcare decisions
and options.
·Gaining patients trust, ensuring they feel cared for and understood.
·Regularly updating medical records and coordinating with the healthcare team for seamless
patient care.
·Taking on additional responsibilities as required for effective patient support.
Requirements:
·Legally authorized to work in the US.
·Active and current Nurse Practitioner license in Arizona.
·One year or more of experience in healthcare or patient management is preferred
(but not required).
·Strong skills in organization, time management, and adaptability to change.
·Excellent communication skills, both in explaining complex information and in actively
listening to patient needs.
·Empathy and teamwork skills are highly valued.
