Front Office Medical Assistant

Description
At Catalytic Health Partners (CHP) our Clinical Support Provider are dedicated to supporting our members to meet our field team in their home focusing on their social, behavioral, and physical needs. Medical Office Specialists will also be working via phone, text messages, and email with members to learn their needs then, working as a team, create plans to help members with those needs. Many of our members need help navigating the healthcare communities, community resources for social needs and maximizing their AHCCCS benefits.
Responsibilities
- Establish working relationships and trust with members by phone to learn the details of their needs and how their needs can be met.
- Request medical records from various facilities and upload documents to member charts. Send medical records to external providers as requested.
- Will oversee the coordination of all members’ HEDIS metrics to monitor the growth and progress of each member as it relates to quality care.
- Will manage HEDIS measures to meet and exceed AHCCCS standards to close gaps in members’ clinical care.
- Will oversee the ordering and delivery of durable medical equipment (DME) to members in a timely manner.
Skills
- Follow-through with the order and delivery of durable medical equipment (DME).
- Send and receive member medical records internally and externally.
- Set up and schedule primary care physician, specialist and dental and vision appointments.
- Coordinate and oversee healthcare effectiveness data and information set (HEDIS)
- Collaborate with CHP Health Plan teams on improving members’ quality metrics
Required Qualifications:
- Legally authorized to work in the US.
- High school diploma or equivalent.
- Experience in a customer service environment. Excellent communication skills, both in explaining complex information and in actively listening to patient needs.
- Ability to multitask among many tasks throughout your day.
Preferred Qualifications:
- Certificate in Medical Assistance.
- Strong skills in organization, time management, and adaptability to change.
- Empathy and teamwork skills are highly valued.
- Ability to prioritize tasks to ensure efficient completion of all needs.
- Working ability in Microsoft products especially Word, Excel, Outlook and OneDrive.
- Ability to navigate computer systems while also being on phone.
