At Catalytic Health Partners (CHP) our Care Coordinators are dedicated to meeting with members in their home focusing on their social, behavioral and physical needs. Care coordinators will be working with members to learn their needs then, working as a team, create plans to help members with their needs. Many of our members need help navigating the healthcare communities, community resources for social needs and maximizing their AHCCCS benefits. CHP is an integrated population management company, and our Care Coordinators work closely with Nurse Practitioners and Behavioral Health Specialists to address the holistic needs of our members. We engage with members who are high cost, high risk and have low engagement with the health care community and our performance is based on how well we as a team address their complex holistic needs. We do not replace their community providers and clinics, but rather help guide members on how to effectively use their community resources. All our team is remote and, in the field, seeing 5 members per day in the member’s home setting for in person visits.
Area Covered
This position is covering the Yavapai East area. This area includes the areas of Cottonwood, Camp Verde, Cornville, Dewey, Jerome, West Prescott Valley
Day to Day
A typical day for a Care Coordinator will begin from their home to their first member’s home setting. You will see 5 members per day with an office team helping to set your recurring appointments with members. You will meet with members for generally about 1 hour addressing their goals, barriers, plans for achieving their goals as well as completing vital signs, medication reconciliation, social needs and depression screenings. After your appointment you generally have a 30-minute time frame to make phone calls, respond to emails and other such administrative needs for your member population before the next appointment. Many times, Care Coordinators are working to find resources to address member’s social needs, helping member’s coordinate healthcare appointments, and ensuring members are taking advantage of all benefits of AHCCCS. In your role you are part of a team all working collaboratively to ensure your members’ needs are met including their social, behavioral and physical needs. Your work schedule is a standard Monday through Friday with flexibility in your appointment times based on member needs and your needs.

Responsibilities
- Interview patients and document basic medical history, vital signs
- Develop an understanding of the patients’ social and physical needs
- Organize and schedule appointments-testing to close gaps in care
- Coordinate and assist patients to advocate and obtain resources
- Work as a team to meet patient needs and plan for patient success
Skills
- Proven experience as a medical assistant or care coordinator
- Knowledge of medical office management and procedures
- Excellent time management skills and ability to multi-task
- Social perceptiveness and service oriented
- Excellent written and verbal communication skills
- Degree in medical assistance, certified nursing assistant, behavioral health tech or equivalent
- Ability to work in a team environment
- Adaptability to changing scenarios and situations
Minimum Requirements Education
Minimum High School diploma or equivalent with a certificate as a medical assistant, certified nursing assistant, behavioral health tech.
Willing to consider others with a background in health care and patient management
