As a Physical Therapist, you will serve as a key partner in each patient’s plan of care, working directly in patients’ homes across St. Peterburg & Clearwater, FL and surrounding areas. Duties And Responsibilities
- Identify and meet patients’ goals and needs through comprehensive assessment and intervention.
- Develop and implement individualized care plans using a variety of treatment techniques.
- Deliver quality, evidence-based, and personalized care to patients in their homes.
- Motivate and support patients during therapy to help them achieve optimal function.
- Promote healthy lifestyles by improving patients’ strength, flexibility, balance, and coordination.
- Evaluate treatment effects, monitor progress, and communicate updates to the care team.
- Collaborate with physicians, caregivers, and other healthcare professionals.
- Maintain accurate and timely documentation of patient care.
- Other responsibilities as assigned.
- Minimum of 1 year of skilled physical therapy experience.
- Proven work experience as a physical therapist, ideally in a home health or clinical setting.
- Licensed/registered as a physical therapist in the state of Florida.
- Ability to manage patients with varying personalities and needs.
- Current knowledge of physical therapy treatment practices.
- Excellent interpersonal and communication skills.
- Demonstrated commitment to ongoing professional growth and learning.
- Professional behavior and strong organizational skills.
- Experience working in home health care.
- Familiarity with electronic health records or home health documentation systems.
- Interest in patient education and community health promotion.
