Compensation from $20.50 - $22.50/Hour - (based upon experience) -
TEMPORARY POSITION
Day 8am - 4:30pm; rotating weekends and holidays
The At Home Services Coordinator/Supervisor is responsible for the coordination of services offered to the Independent Living, Assisted Living, and Skilled Nursing residents related to physical, social, emotional, psychological and recreational needs. Supervises at least two or more team members.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- High School diploma or GED required
- Minimum two years’ experience in a related health services field required
- Knowledge of Microsoft Office, including Word & Excel, and email applications required
- Must possess excellent written and verbal communication skills
- Demonstrated organizational skills with the ability to prioritize work assignments
- Must communicate effectively with residents, family members and team members
JOB ACCOUNTABILITIES:
- Responsible for the coordination and scheduling of At Home Services.
- Responsible for the recruitment, interviewing, hiring, training, supervision and implementation of corrective action for At Home Services team members.
- Prepares At Home Services team member schedules within budgetary guidelines, providing adequate team members to assure quality of care and to meet resident and community needs.
- Coordinates Independent Living resident services with other team members to maximize services for all residents.
- Develops positive relationships with Independent Living residents, family members and other constituent stakeholders of the community.
- Consults and meets with community team members through Continuum of Care meetings on matters related to Independent Living residents.
- At least annually, evaluates job performance of At Home Services team members.
- Maintains knowledge of resident conditions through the At Home Services team members and personal visits.
- Conducts regular team meetings for purposes of planning, coordinating and direction of Independent Living resident services.
- Coordinates processing of Independent Living resident information with administrative team members including, but not limited to resident file database entry and maintenance.
